Category	Allow selection from Biological, Chemical, Electrical, Extreme weather, gravity, Location, Machinery and Equipment, Manual Tasks, Noise, Psychological, Radiation.
Description	Allow free text entry.
Photo
Active?	Allow specifying whether the hazard is active.
Other Attachment	Allow attaching a document.

Corrective action will permanently fix the hazard?	Allow selection from 'Yes' or 'No'
Potential harm	Allow free text entry.
Uncontrolled Risk	Displaying the level of risk low, medium or high.
Score	Displaying the calculated risk score based on two dimensions.
Indicator	Displaying color corresponding to the level of risks.
Significant Safety Hazard?	Allow specifying if the hazard is a significant safety hazard. If checked an additional section shall appear to allow conducting a controlled risk assessment.
Dimension	Displaying two dimensions severity and likelihood.
Degree	Allow selection from three levels low, medium or high.
Score	Displaying score corresponding to the degree.
Guidance	Displaying instruction on conducting the evaluation.
