In order to link an Action button with a Mail Template ensure that the Mail Template is Enabled. 


Proceed to the form in the Form Builder, open the settings of the Action Button -> Select 'Create Email' under Button Actions -> Select the enabled Mail Template

Submit the field changes and proceed to Publish the form to make the changes permanent.
